**Title – a descriptive title of the dataset

**Description – The what, why, when, where and how aspects of the data

Known uses of data – Brief details about downstream consumers of the data (if known)

License (dropdown) – License definitions of data

Organization (dropdown) – Organization that is providing the data, from list of defined organizations

Source – Source of the data

Update Interval (Months) – A metric of how often the data is updated

*Data Owner Organization – Organization that is responsible for the data

*Data Owner Contact Name – Name of person or role responsible for the data

**Data Owner Contact Email – Email address of person or role responsible for the data

Data Collection Organization – The organization that collected the data

Data Collection Contact Name – Name of person or role responsible for the collection of the data

Data Collection Contact Email – Email address of person or role responsible for the collection of the data

Data QA/QC Organization – The organization that QA/QC’d the data

Data QA/QC Contact Name – Name of person or role responsible for the QA/QC of the data

Data QA/QC Contact Email – Email address of person or role responsible for the QA/QC of the data

Funding Agency Organization – The organization that funded the acquisition of the data

Funding Agency Contact Name – Name of person or role responsible for the funding of the data

Funding Agency Contact Email – Email address of person or role responsible for the funding of the data

Coordinate System – The geographic coordinate system used in the dataset

Data Collection Procedures – Brief details about how the data was collected, including protocols and DOI/URL if possible

Data Quality Procedures – Brief details about how the data quality was reviewed, including protocols and DOI/URL if possible

Tags – A list of keywords to help classify the dataset         

Version – Version number of the dataset (default is 1.0)

Custom Field(s) – Users can add up to three with a Key(word) and Value

Data – Choice of upload button or link button to data

 Name – Name of resource

Description – Brief description of resource

Format – Resource format, (ie csv, html, json, zip)
